She married Charles Banks in 1961.  She later married Phillip Ward in 1976.  She then married Dennis Cook August 11, 2003 at Las Vegas, Nevada.

She worked in retail for Revco and Dillons until retiring.

Mrs. Cook was a Great Bend, Kansas resident since 1976 coming from Carrollton, Texas.

She was a member of First United Methodist Church, Great Bend; and Veterans of Foreign Wars Post 7428 Auxiliary, Hoisington.

two sisters: Betty Palmer of Glenwood, IA and Sue Jaske of Great Bend, KS

Grandchildren: Nine, including Heather Madrid of Great Bend, who she raised

Great-grandchildren: Three

 

She was preceded in death by step son, Shawn Cook, daughter, Cara Stevens, and two brothers, Bobby Fahlsing and Ronnie Smith.

Place of Service: Bryant Funeral Home

Officiant: The Reverend Lennie Maxwell

Inurnment: Hillside Cemetery, Kinsley, Kansas at a later date

Visitation:  The family will receive friends from 6:00 to 8:00 p.m., Monday, September 15, 2014 at the funeral home
